Actually the most common cause is none of these. Falls, which occur much more frequently in Ohio factories, are the chief culprit. Except for traffic accidents, falls are the principal cause of injuries.

The Division of Safety and Hygiene of the Industrial Commission of Ohio has listed the major causes of falls and how to prevent them.
TRIPPING OVER OBSTACLES: Maintain good housekeeping.
FALLS FROM LADDERS: Avoid defective ladders; use safety feet: foot of ladder should be 1/4 of height from base of wall.
FALLS FROM ELEVATED LOCATIONS: Use safety belt and tieline.
FALLS FROM SCAFFOLDS: All scaffolds should have hand rails and toe boards and be securely mounted.
SLIPPERY SURFACES: Avoid oil spills; use special care when slippery surface is unavoidable.
UNSAFE SUBSTITUTES FOR LADDERS: Use a ladder instead of boxes, chairs, open drawers, etc.
On the job and at home always be alert to falling hazards. Remember, the falls don't hurt it's the sudden stop!
